Polycystic Ovarian Syndrome (PCOS) is a prevalent endocrine disorder among women of reproductive age, marked by hormonal imbalances, irregular menstruation, and psychosocial complications that adversely affect health-related quality of life (HRQoL). This study sought to assess the knowledge and HRQoL of women diagnosed with PCOS in Jolo, Sulu, an area characterized by limited healthcare resources and reproductive health awareness. Using a descriptive quantitative research design, data were collected from 40 clinically diagnosed women through a validated questionnaire measuring socio-demographic data, PCOS-related knowledge (causes, pathophysiology, complications, and management), and HRQoL (impact, emotional, mental, physical, and menstrual dimensions). Results indicated that respondents possessed a moderate level of knowledge, with higher familiarity on causes and complications but lower understanding of pathophysiology and medical management. Despite knowledge gaps, participants demonstrated a relatively high HRQoL, especially in coping with menstrual problems and emotional stressors. Notably, statistical analysis revealed significant associations between educational attainment and both knowledge and HRQoL, as well as a positive correlation between PCOS knowledge and quality of life outcomes. These findings underscore the critical role of reproductive health literacy in shaping women’s experiences with PCOS and highlight the need for culturally responsive health education programs. This study advocates for integrated health strategies that combine clinical care, psychosocial support, and community engagement to empower women in marginalized settings to take proactive control of their reproductive and mental health.
